Passages Camper Information Form
Please complete all required fields for each camper in your household. This information is used directly by our administrative and leadership staff to prepare for emergency contact, health clearance, and daily camp operations.
To ensure a seamless camp experience, Passages requires all participants to submit updated registration forms each year prior to the start of their session. Even if your child attended camp in previous seasons, new copies of all forms must be completed before arrival.

Camper Information and Medical Form: Includes essential camper background details, counselor notes, and pickup authorizations, as well as provides updated health history, allergy details, emergency contacts, and physician care details.

Assumption of Risk Form: Outlines safety disclosures and legal acknowledgments required for program participation.
