Assumption of Risks

INSTRUCTIONS FOR PARENTS: 
ADULT NAME -and- ADULT BIRTHDAY must be entered first.
Child's information will be entered later

IMPORTANT!
1) Waivers for minors must be signed by a PARENT (or COURT APPOINTED legal guardian).

2) Waivers for minors signed by a friend, relative or group leader WILL NOT BE ACCEPTED.

3) For adults who are unable to complete the form on their own, a supervising adult may complete the waiver for them by selecting Supervised Adult. Similar to the instructions for parents above, the first set of adult fields should be completed with the supervising adult's information. Any Supervised Adult waivers submitted for an adult that does not fall into that description WILL NOT BE ACCEPTED.
